In Minnesota, we are currently grappling with yet another measles outbreak, a danger that is entirely avoidable with adequate vaccination rates. This is the reality of a post-COVID era defined by politicized basic healthcare and well-funded anti-vaccination groups striking fear in well intentioned parents. Joining together with patients and families in pediatrics can present challenges, particularly when it involves convincing adolescents to follow medical recommendations. There are times when less than ideal compliance to a care plan is undertaken. This can leave clinicians in a confusing situation where they may not know what to do; where they are unsure how to do good for the patient and respect the adolescent's assent and dissent. This can lead us to having to consider a "least worst" option or path forward. As all pediatricians know, children are not just "little adults" and this is especially true in the field of ethics. In matters from consenting to surgery to deciding end-of-life wishes, adults are assumed to have decisional capacity until proven otherwise. Adults have the benefit of years of developmental maturation and experiences that have shaped a unique set of preferences and underlying values. Children, on the other hand, pose a more complex picture. Given their wide range of cognitive abilities (from neonates in the NICU to frontal lobe deficient teenagers), decisional incapacity is the default and deciding "best interest" is often in the hands of mom and dad. This presents obvious conflicts in many situations: from refusing vaccines to reckoning with an adolescent who has decided they're done with chemotherapy and its side effects. Preterm birth affects approximately 1 of every 10 infants born in the United States, and almost all of those infants will end up spending time in a Neonatal Intensive Care Unit or NICU. For many, this involves several days of infants receiving care in a hospital, with parents unable to take their babies home, disrupting their family and work life. Often the first question families ask their NICU care teams is, “When can I go home?” Recently, Children’s Minnesota launched the Children’s Home Application-based Monitoring Program, which is a one of it’s kind program in Minnesota that allows infants that qualify to receive expert care and monitoring by the baby’s neonatal care time at home, with overwhelmingly positive results. It’s no secret that we are facing a mental health crisis among our pediatric patients. Kids and teenagers have higher rates of depression and anxiety than ever before, and suicide has now become the second leading cause of death for adolescents. More and more of our patients and families are seeking mental health care support and services, and we have an unfortunate shortage of pediatric and adolescent mental health specialists to fill the need. This has resulted in kids showing up in clinics and emergency departments in crisis, and sometimes spending days to weeks sitting in a hospital awaiting specialized inpatient mental health care. A mysterious wave of acute hepatitis in children is currently under investigation spanning 43 states as well as international borders. These cases are unique in that typical causes for liver injury such as viral Hepatitis A-E have not been found. Though the causal pathway for liver injury is unknown, association with a certain virus of cold-causing daycare fame has been demonstrated in more than half of patients: adenovirus, particularly F type 41. Worldwide most children have fully recovered though around 10% have needed liver transplants and the vast majority have been hospitalized. Is it a bird? A plane? A mobile simulation bus? Mobile simulation, which brings vehicles equipped with spaces to simulate patient care areas, task trainers, and mannequins directly to the healthcare provider, has steadily increased over the last decade. Since 2007, the Children's Minnesota Mobile Simulation Center Bus has traveled to provide hands on practice for healthcare professionals across the region. The Mobile Simulation Center is a 40 foot RV outfitted to look like a hospital space, and designed for training teams in the field. Like Zoom birthdays and designer masks, multi-system inflammatory syndrome in children (MIS-C) is a unique product of the COVID-19 pandemic. Appearing about 4 weeks after COVID-19 infection has cleared, it likely represents a disordered immune response that can cause appendicitis-like GI symptoms, Kawasaki-like skin findings, and most alarmingly, cardiac dysfunction and pump failure. Recognizing MIS-C’s presentation and having a diagnostic approach in mind will be increasingly important for pediatricians as we enter Omicron’s afterglow. It's National Heart Health Awareness Month! Congenital heart defects are the most common birth defect among the kids that we care for, and perhaps contribute to some of the most fear inducing moments in the newborn nursery. The good news is that many congenital heart lesions are treatable with very good outcomes, if detected and treated early by cardiac specialists. As those of us in clinical practice know, we have seen soaring rates of mental health challenges among children, adolescents and their families during the course of the COVID pandemic, exacerbating the already tenuous situation that existed for child and adolescent mental health. We have seen rates of childhood mental health concerns and suicide rise steadily over the last decade. Across the country, we have seen dramatic increases in ED visits for acute mental health emergencies, including suspected suicide attempts. Almost two years into the COVID pandemic, and so much has changed for kids. For some of our young patients, they can't remember a time without masks, social distancing and life online. What impact has this "new normal" had on kids, particularly those whose worldview and social interactions have been shaped primarily by the pandemic? Join our conversation with Kathy Hoogerhyde, student support psychologist, as we discuss how the social and emotional development of kids have been impacted by repeated quarantines, social distance guidelines and a lack of typical group social interactions. Learn about factors that impact childhood resiliency, the social developmental tasks that have been impacted for some of our youngest patients and the lack of coping strategies that are becoming more and more apparent in school-aged kids. When speaking about neutropenia, most pediatricians will recall oncology patients they cared for in residency who were floridly septic, mucosal sores evident, and too vulnerable to infection to safely take a rectal temperature. Outside of oncology, however, neutropenia has many "flavors" and is often an incidental lab finding in an otherwise healthy, smiling child. While understanding every possible cause for neutropenia is outside the scope of today's episode, we will focus on major findings that offer insight into two important questions: (1) is this patient at risk for serious infection? And (2) do these findings point to a dangerous underlying pathology that needs to be explored further?

Not yet a decade into his career, Dr. Nate Chomilo is already blazing trails through adovcating improved childhood literacy, raising awareness about social determinants of health and tackling the tough problem of racial health inequities. On this Trailblazers episode, we learn about Dr. Chomilo's early advocacy work in residency, as he found a passion for the social determinant of health of childhood literacy, through the Reach Out and Read program, where he would eventually become the Medical Director for Minnesota. This led him to broader work with the Minnesota Governor's Early Learning Council and eventually into his current position as Medical Director for the State of Minnesota's Medicaid and MinnesotaCare programs. Through his work with the State of Minnesota, as well as co-founding the Minnesota Doctors for Health Equity, Dr. Chomilo shares with us his passion for early childhood intervention and healthcare access and how clinicians and health systems can address racial and health equity.

All providers are familiar with long, dubious allergy lists pointing to a rash around the time a medication was taken. Given the endless list of competing causes for rash, misattribution to medications is wildly common as many studies by pharmacists have shown. The result is “allergy jail”, where angry pop ups limit our treatment options, impact antibiotic stewardship, and often follow a child into adulthood. According to recent research, 85% of teens have at least one active social media profile and almost half of teens are online “almost constantly”. These biases might not be the same as our conscious beliefs, but they can influence our actions and perpetuate children’s health disparities. Primary care doctors frequently encounter kids with episodic, crampy leg pain, often worse at night and felt in deep muscular compartments bilaterally. So-called “growing pains” are poorly understood (and likely have little to do with actual linear growth) but thankfully self-resolve in time. Through this case, we discuss when reassurance for growing pains is warranted, what else is on the differential and when to be concerned, as well how continuity with families informs medical decisions and the evaluation of "sick" versus "not sick".

Almost one year ago, the world watched in horror as former Minneapolis police officer Derek Chauvin knelt on the neck of George Floyd, ultimately taking his life. After this event, and in the middle of a global pandemic, we both watched and participated in a global awakening and uprising for racial justice, with many heeding the call to take action to end systemic racism as they never had before. While Derek Chauvin's trial gets underway and we prepare for the one year anniversary of George's Floyd's death, we sit down with our guest host James Burroughs, VP and Chief Equity and Inclusion Officer at Children's Minnesota, to talk about what it's been like to continue the work of advancing health equity and inclusion over the last year, what progress has been made, what work there still is to be done. 2021 is shaping up to be one of the worst years for anti-LGBTQ legislation to date. So far over 175 bills have been introduced in 32 states that seek to restrict the rights of LGBTQ individuals, with several of them specifically targeting transgender and gender diverse kids. So, what should pediatric clinicians be looking for when it comes to signs and complications of COVID in children? One thing we know for sure is that "COVID clots". As of January 2021, there were over 1600 total reported cases of multisystem inflammatory syndrome in children, or MIS-C, and 26 pediatric deaths. Given that most cases of MIS-C occur between the ages of 1-14, and kids under 16 are not yet eligible to receive the COVID-19 vaccine, understanding how to recognize and respond to MIS-C is something that everyone working in pediatrics needs to know. We are in the middle of a public health crisis. At almost a year into the COVID-19 pandemic, infection, hospitalization and death rates are skyrocketing toward all time highs. This weeks numbers suggest that nearly two people died from COVID-19 every minute this past week. Infection rates are high, our hospitals are filling up and as the weather is getting colder, it is forcing us indoors and potentially into more exposures with upcoming holidays. It appears that the light at the end of the tunnel for all of this is the rapid development of a vaccine.
