Postmenopausal bleeding can be an alarming experience, especially when you've gone years without a period. What causes this unexpected bleeding? When should you be concerned? Dr Ceri Cashell joins me to demystify this common but often distressing symptom. We dive into the definition of postmenopausal bleeding - any bleeding that occurs after 12 months without periods - and explore the various causes, from hormone imbalances to more serious conditions that require immediate attention. Dr Ceri uses a brilliant analogy, comparing the uterus to a jumbo jet where multiple hormone "switches" need perfect alignment for smooth operation. For women recently started HRT, bleeding can be a side effect, particularly in the first three to six months or after dosage adjustments. However, Dr Ceri emphasises that certain risk factors like obesity, family history, tamoxifen use, and smoking warrant prompt investigation. We walk through exactly what happens during medical assessment - from initial GP visits to ultrasounds that measure endometrial thickness and potentially hysteroscopy procedures. My own experience with unexpected bleeding after eight years without periods highlights how confronting this symptom can be. Dr. Ceri reminds us that while HRT offers tremendous benefits, it's not a perfect solution and may require personalised adjustments to find your hormone sweet spot. Most importantly, she urges women not to dismiss or normalise symptoms that could indicate something serious - "Women are far too good at sucking things up," she notes, encouraging listeners to prioritise their health concerns. Have you experienced unexpected bleeding after menopause? Don't wait - speak with your healthcare provider today and get the answers and reassurance you deserve. Have you ever wondered why weight creeps on during menopause, despite your best efforts? Dr Lucy Burns pulls back the curtain on what she calls the "metabolic triad of menopause" – a perfect storm of declining estrogen, rising cortisol, and increasing insulin resistance that creates the ideal conditions for stubborn weight gain. As a GP who retrained in lifestyle medicine after experiencing her own health challenges, Dr. Burns offers a refreshingly honest perspective on why traditional approaches to weight management fail so many midlife women. Using the powerful analogy of a locked woodshed, she explains how high insulin levels essentially padlock your stored fat, making it impossible to access no matter how much you restrict calories or exercise. What makes this conversation truly valuable is how Dr Burns connects the dots between hormonal changes, metabolic health, and practical lifestyle solutions. She introduces her "six S's for success" – sustenance, sleep, stress management, strength training, sunshine, and social connection – as foundational pillars for reclaiming metabolic health during menopause and beyond. The discussion takes fascinating turns through hormone therapy considerations, the psychological aspects of food addiction, and why so many of us grew up alternating between strict dieting and giving up entirely. Throughout, Dr Burns maintains that improving metabolic health should be the primary goal, with weight loss simply being "a happy side effect" of addressing the underlying hormonal imbalances. If you're exhausted by diet culture but still concerned about your health during menopause, this episode offers a science-based approach that works with your changing physiology rather than against it. Discover why muscle is your "metabolic organ," how stress impacts your ability to lose weight, and practical steps to break free from the metabolic challenges of midlife. Ready to stop blaming yourself and start understanding the real science behind menopausal weight changes? Listen now and discover why it's time to focus on metabolic health, not just the number on the scale. The relationship between menopause and neurodivergence remains largely unexplored territory in women's health—until now. In this groundbreaking conversation, neuropsychologist A/Professor Caroline Gurvich shares fascinating insights into why cognitive symptoms during perimenopause may signal more than just typical hormone fluctuations. For many women, perimenopause becomes the unexpected moment when long-overlooked neurodivergent traits suddenly demand attention. "About 98% of people said their ADHD symptoms became worse across those perimenopausal years." Even more revealing, numerous women receive their first ADHD diagnosis during midlife, despite having experienced symptoms since childhood. This raises questions about why these conditions often go unrecognised, particularly in women, until hormonal shifts disrupt longstanding coping mechanisms. Listen in to learn more about Estrogen's protective effects on dopamine - a neurotransmitter central to ADHD - which diminishes during perimenopause, potentially unmasking symptoms that were previously manageable. Beyond ADHD, A/Prof Gurvich discusses how autism, dyslexia, and other neurodivergent conditions may present differently during menopause, and the compelling connections between PMDD, perinatal depression, and neurodivergence. Whether you've wondered about undiagnosed ADHD, experienced increasing cognitive challenges during perimenopause, or simply want to understand the fascinating intersection between hormones and brain function, this episode offers invaluable insights and practical guidance. Listen now to better understand your changing brain and discover strategies to thrive during this transformative life stage. Supplement confusion is at an all-time high, with social media feeds flooded with miracle promises targeting menopausal women. But how do you know what you actually need, what works, and what's just expensive snake oil? Pharmacist and nutritionist Sarah Gray brings her dual expertise to this conversation, offering practical guidance for navigating the overwhelming world of supplementation. Unlike most health professionals who specialise in either conventional medicine or natural approaches, Sarah bridges both worlds, providing uniquely balanced insights into what supplements might actually benefit women in midlife. This episode delivers specific, actionable advice you can use immediately. Learn the simple trick for identifying quality supplements (look for the AUST-L or AUST-R code), understand which baseline blood tests you should request from your doctor, and discover why blindly following supplement trends can waste your money and potentially harm your health. Sarah warns against the common practice of continuously adding supplements without removing any, explaining how this can lead to accidental overuse of certain nutrients. We take a deep dive into the essential blood work every midlife woman should request annually. Sarah explains how certain symptoms commonly attributed to perimenopause could signal nutritional deficiencies that won't necessarily improve with hormone therapy. And you don't want to miss our discussion about Creatine - arguably the most evidence-backed supplement for women in midlife - which offers benefits for muscle strength, mood regulation, and the immune system. Take control of your health by understanding exactly what your body needs rather than what clever marketing suggests you buy. In this popular episode, Dr. Talat Uppal, an obstetrician-gynecologist specialising in heavy menstrual bleeding, shares her expertise on a condition affecting one in four women yet rarely discussed openly. This eye-opening conversation sheds light on how women often normalise suffering through periods that drastically impact their quality of life. The discussion reveals shocking statistics: 25% of women experience heavy menstrual bleeding, yet more than half never seek medical help. Dr. Uppal provides clear indicators of abnormal bleeding – passing large clots, changing protection every 1-2 hours, waking at night to change, using multiple protection types simultaneously, and embarrassing "flooding" episodes. These symptoms can lead to iron deficiency, anaemia, and significant quality of life impacts. Learn about the spectrum of treatment options, from medications like Tranexamic Acid to the highly effective Mirena IUD and surgical interventions including endometrial ablation. She emphasises that most women find significant relief once properly treated, with many asking, "Why didn't I have this earlier? Why did I suffer so much?" Perhaps most powerful is the exploration of how heavy bleeding becomes normalised at multiple levels – by women themselves, healthcare providers who don't offer full treatment options, and society that suggests women should simply endure until menopause. This normalisation condemns many to years of unnecessary suffering when effective solutions exist. The conversation offers practical advice for women seeking help, emphasising the importance of advocating for yourself and being knowledgeable about available options. Have you been suffering in silence? This conversation might be the catalyst for transforming your quality of life. Listen, learn, and advocate for your menstrual health. What happens to our sexuality during menopause? Far from signalling an end to pleasure and intimacy, this transition offers an unexpected opportunity for sexual rediscovery and deeper connection. Psychologist and sexologist Laura Lee joins me in this episode to masterfully unpack the evolution of desire through midlife, challenging our cultural fixation on "loss of libido" by revealing how desire simply changes form. Sexual communication can feel particularly challenging during menopause, when changing bodies and shifting desires create vulnerability. Laura provides practical strategies for starting these conversations without the paralysing pressure many couples feel. Rather than dreading "the talk," she suggests sharing resources, expressing genuine curiosity, and validating one another's experiences to foster meaningful dialogue about evolving needs. Perhaps most thought provoking is Laura's invitation to expand our sexual menu beyond the narrow, penetration-focused definition of sex that dominates cultural narratives. By embracing a broader understanding of sexual connection—from sensual touch to passionate kissing—we create more opportunities to say "yes" to pleasure, even as our bodies change. This expansion allows us to discover new pathways to pleasure uniquely suited to our midlife bodies. Ready to transform how you think about sex during menopause? Listen now to discover how this transition can become not an ending but the beginning of your most authentic sexual experience yet. Links: Laura's website Thank you for listening to my show! Buckle up, as we, Sonya and Roma, challenge the fear-based narrative often associated with perimenopause and menopause, particularly on social media. Worried that you've started drinking too much but know that you're still a long way from needing an intervention? Rachael Layton went through years of questioning her relationship with alcohol, knowing that she was drinking too much and worried that she was heading down a dangerous path. While she knew she wasn’t an alcoholic (although that didn’t stop her from occasionally Googling “Am I an alcoholic?”), she also wasn’t someone who was happy with just a glass or two every now and then. So, what do you do when you feel that alcohol is doing you more harm than good, but you haven’t hit a ‘rock bottom’ where the booze is controlling your life? You discover the land of the grey-area drinker, and you might be surprised to know that a large percentage of drinkers fall into this category. When Rachael got to the stage where she didn’t like the direction her drinking was headed, she started looking into why alcohol had such a hold on her. Leading her to learn how to make alcohol small and irrelevant in her life, which has been a game changer. It’s led to better physical and mental health, new and exciting opportunities, and a whole world of positives that Rachel had not expected. In this episode, you will hear us discuss: How Rachael became concerned that alcohol was impacting her sleep, health, body and mood more than her perimenopause symptoms The mindset shifts that Rachael learned helped her rethink her drinking Why Rachael embarked on a career change so she can help other women change their relationship with alcohol too Resources Rachael's website Instagram Other Episodes You Might Enjoy Normalising sobriety with Sarah Rasbatch Mindful drinking & challenging the norm with Irene Falcone Thank you for listening to my show! Professor Kulkarni's top tip: Women need to back themselves when it comes to their mental health. When they know intuitively that they are not OK and that they need help, to have the courage to keep asking for help, to keep pushing back until they are heard and taken seriously. You know yourself better than anyone. Have faith that you know what's right for you. "I think that there's a great missed opportunity for trainers, professionals, nutritionists, and coaches to step down off of Mount Olympus and mingle with the mortals." Ren Jones was raised in a multi-generational female household and heard all the conversations that young men don't usually get exposed to. He learned that there was a cloud of shame associated with menopause and how it was seen historically. He experienced a powerful reminder of his own mortality when he lost his brother, sister and mother to heart disease, drinking & driving, and cardiovascular issues respectively. This tragedy led Ren to pursue a career in fitness and wellness, and with it, the realisation of the lack of information available for women's health in comparison to that for men, and hand in hand with that, a lack of empathy. As a result, Ren dedicated himself to learning the language, reading books, and listening to his mentor's advice on cultural humility in order to understand and empathise with the life he could never live. A women's life. With an open heart and a dedication to understanding, Ren has since been able to better comprehend the subtlety of the female experience and help others do the same. In this episode you will learn: 1. Why Ren took a deep dive into the lack of research and resources available to women, now coaching male trainers on their need for more education and understanding of the complexities of women's health. 2. The Power of Human Connection: the impact of sharing one's story and understanding the importance of cultural humility when working with women. 3. The Historical Significance of Menopause: examining how the patriarchy has viewed menopause historically and its influence on the perception of the issue today. Ren Jones is an online trainer and advocates for women's health, based in North Carolina. He is passionate about educating male trainers on the complexities of training women and creating a more inclusive fitness and wellness industry. "Break the stigma, take a stand - join the movement to normalise being human in the workforce!" "Share from the scar, not from the wound" - Lou Johnson (not the original source) Lou Johnson is an almost 55-year-old publishing professional with over 30 years of experience. She is a feminist and advocates for normalising conversations around menopause, menstruation, motherhood and more in the workplace. Lou wrote an article about menopause, menstruation, life, and work to process her own feelings and to share her lived experiences. The article was published by Women's Agenda and sparked many conversations about the topics, prompting women to reflect on the shame they have experienced and how they could normalise the conversations for others. Through the article, Lou wants to give permission for women to be human both in the workplace and in life and to challenge the traditional gender norms. In this episode, you will learn the following: 1. What is the power of normalising conversations around menopause and menstruation? 2. How can we create permission to be human in the workplace? 3. What are the ways that gender norms can be challenged and changed? Find out why I'm chatting with a middle-aged man with a passion for perimenopause and menopause. "I think that the constant challenge for people working in science and medicine in particular is the way that that evidence gets translated. And I think that what you've mentioned before is a classic example of research not being communicated well that has such profound implications for women everywhere of that age." Phil Hayes-St Clair is a business leader and advocates for women's health. He is the founder of Drop Bio Health and is a passionate educator, working to close the global education gap around perimenopause and menopause care. Phil grew up in Australia and served in the army, but left due to a medical issue. He started his own business, Drop Bio Health, to bridge the gap between medical research and people's knowledge of their own bodies. In his recent Ted Talk, he discussed the need for more comprehensive education on women's health, especially for those in rural and remote communities. He highlighted the need for knowledge and empathy around perimenopause, menopause and aging to ensure that women have access to the care they need. Through conversation, Phil hopes to normalize the conversation around women's health and honour the women who have gone before. In this episode, you will learn the following: 1. The need for better accessibility to women's health care, especially for those in remote and rural areas 2. The importance of communication in relationships, particularly around health issues 3. How do we change the narrative around menopause in the workplace? Professor Rebecca Mitchell, of Macquarie Business School, is leading a study that draws together Macquarie Business School’s Health & Wellbeing Research Unit and Psychological Sciences researchers of the Faculty of Medicine, Health and Human Sciences, to answer that question. The project explores the intersection of menopause and women’s careers through a positive psychology lens. Professor Mitchell says... "women are to some extent on their own when it comes to managing menopause. There are big gaps in the research around how women, as leaders, progress through this period." “There's nothing [in the literature] about how women can become more effective, more productive, how they can change careers, be adaptable, transition into new ways of working; there's nothing about that at all." “Nobody wants to talk about it. And that's amplified by the fact that people don't know what to say, because there's this dearth of research to guide them. There's a skerrick of research that looks at what can be done, but a lot of the solutions are generic: time off, education and training, and so on.” Expect to learn a lot in this episode with Rebecca about exactly what it takes to introduce policy change into workplaces. It's a lot more complicated than I initially realised. Topics we discuss: The questions workplaces and leaders should be asking, but aren't Why announcements around additional sick leave are tokenist, only providing a stop-gap solution Why many women give up on their leadership and career progression goals as a result of menopause What research is being done, how and how long it will take How you can be involved in the research LINKS Email: rebecca.mitchell@mq.edu.au About Rebecca RELATED EPISODES Prof Jayashri Kulkarni: Mental Health and Menopause Thank you for listening to my show! Has menopause led to you shutting up shop on your sexuality? A few years ago, Susan Jarvis (in her own words) went from being a very horny, lustful ‘wench’ to someone who had virtually shut up shop on their sexuality. She once thrived on being a hot-blooded woman, it gave her confidence and energy knowing that she had a lust for life and lived a life with passion. Wearing red lipstick every day with pride, Susan went from feeling euphoric about going to work wearing the smile on her face from the previous night’s orgasm to dragging herself in and feeling like a dried-out, old horn bag with a flat battery. ** WARNING: this episode comes with a Not Safe For Work, Listening in Public or Children warning. Def get the headphones out! Susan considers herself to be an ordinary woman who discovered her extraordinary self by tapping into her sexual energy. Susan shares her wisdom... "I have experienced a transformation from a stale and empty life, to one full of love, passion, laughter and yes, bloody good sex. All because I had the guts to try. The guts to step outside my boundaries and the guts not to care what people thought of me. This doesn’t mean that I went on an escapade of sexual conquests and swinging from the rafters. Rather, my journey was about listening to my intuition, developing self-awareness and improving my communication and interpersonal skills. I modified my perceptions of self and shifted my mindset from being conservative to being adventurous. In other words, letting go of a lot of crap in my life. I am passionate about helping other women, and men, find their extraordinary self, to embrace their sexuality. In particular, I feel extremely frustrated that persons aged over 50, and people living with a disability and/or chronic illness are not represented in the landscape of ageless and positive sexuality. We are alive, and we desire, and deserve sexual fulfilment! In this episode, we discuss Susan's role as The Maven and her online business The Spicy Boudoir. We also breakdown why Susan believes that education, masturbation and communication are the secrets to great sex. Have you started feeling self-conscious at the beach? Are you covering up in a baggy old T-Shirt or worse, stopped going to the beach? Lyndal Sterenburg did too and one summer's day had a spark of an idea that would go on to change women's lives. Today Lyndal has a simple mission - for all women to enjoy summer. And through her business Es Una, she's doing exactly that. Through conversations with friends and other women, Lyndal realised that we all have the same concerns, especially as we get older. Our bodies change, our bellies get bigger, our skin sags … and we become increasingly aware of the signs of ageing, particularly in a Western world that only praises perfection. So, we cover up and ultimately stop doing the things we used to love because we don’t feel bikini ready. And this made her sad. Because Lyndal wanted to feel as good as she felt about going to the beach when she was younger. She pivoted out of the corporate world to dive into a whole new world of fashion design and production, launching Es Una. Offering a range of stylish swimwear, designed for your skin, your curves and your confidence. Es Una has tops, pants and dresses to wear over your bathers like a rashie. As a woman in her 40s (or older), do you want to learn how to exercise in a way that moves you forward but also honours your body through day-to-day fluctuations? Olivia Park is a women's health and performance coach who helps active women lift hard and live well both in and out of the gym. She helps women find liberation from the ‘shoulds’ in fitness and the all-or-nothing mindset through purpose-driven training. And she's a deadset legend. In this episode, Olivia shares about her own hormonal battles. And she answers questions like... How has the toxicity of the fitness industry and diet culture damaged women's relationships with food and exercise? How can exercise be designed from a health-first approach What is LEA (low energy availability), and what are the long-term impacts on women, specifically women over 40. Topics discussed include: Exercise during perimenopause and menopause How to know if your exercise is working for you, not against you Creating positive change and shifting the direction of the fitness industry Exercise with a health-first approach The fitness industry is doing a disservice when the messages are ‘train harder, eat less’. So many women are doing their absolute best to follow this prescription because there’s not enough evidence to prove that there could be a different way. We’re bombarded with sexy sounding F45 shreds and challenges and 1200 calorie diets that are going to make our lives better and finally, we will have the confidence and feel the worthiness we’re promised. Nope. A hormonal and health shit show? Most likely, yes. AND You need enough food to meet the demands not only of your exercise but of living. You need nutrition to support your training, your lifestyle, the stress of being a mum, an employee, an entrepreneur, a solo parent, a caregiver, a partner, a friend … all those stressors on top of training add up. During a career in medicine that included time with the Royal Australian Air Force, Dr Kelly Teagle developed an interest in women’s medicine with further studies and experience in reproductive and sexual health. Ironically, at the age of 42 she experienced an early menopause herself. Her symptoms were debilitating but very responsive to treatment, which led her to realise the importance of getting access to timely advice and care during menopause. Dr Teagle also noticed that her services could be largely delivered just by talking to women, and so the idea for a Telehealth menopause clinic was born and WellFemme was founded in late 2018. WellFemme exists to ensure that ALL Australian women have access to evidence-based menopause and perimenopause care. Kelly and her team achieve this by: Delivering Telehealth peri/menopause consultations Australia-wide, Recruiting and developing a team of caring and motivated menopause doctors, Providing high- quality, evidence-based menopause information through their website, webinars and social media channels, Lobbying for continued Medicare Telehealth rebates for services provided by GP-subspecialists in reproductive and sexual healthcare, Increasing menopause awareness and training for healthcare professionals, and Promoting and facilitating conversations to educate and empower women. WellFemme’s team of doctors are passionate about helping menopausal women reach their best potential. “Shared experiences are empowering experiences” says Dr Kelly Teagle. “This happens when we embrace and discuss what’s happening to us. Normalising menopause breaks down the fear and frees women to explore the potential of their “new normal”. Luan is encouraging people to use Strava (the free digital exercise app) to exercise along a route, making GPS ‘art’ - known as Strava Art - that visually represents BRA Day and share it with the official BRA-va Art Instagram account. This is the first time in Australia that Strava Art has been used in this way and Luan is so excited about the possibilities it presents to pave the way for growing the campaign in future years. Dr Lina Safro and I sat down to discuss the management and treatment of menopausal symptoms, specifically we talk about MHT or menopausal hormone therapy. We also touch on the Women's Health Initiative (WHI) clinical trial findings which severly disrupted the use of MHT's in 2002 and continues to cause fear amongst some women and practitioners today. We discuss who should consider MHT, how long it can be safely taken for and what lifestyle factors should be taken into consideration before MHT is prescribed. We also talked about how having a Mirena impacts menopause. Dr Safro is a GP with a longstanding focus on women's health. Professor Jayashri Kulkarni is one of Australia's leaders in research and treatment of menopausal depression. I believe this episode, is the most important episode I have been able to bring to my Dear Menopause audience. Professor Kulkarni speaks about mental health and the part a women's hormones play in this area, in a way that makes the topic easy to understand and relatable. We discuss why the fluctuation of hormones during the menopausal transition has such a significant impact on mental health, how to recognise if you are being impacted by this and what can be done to support you during this time. We discuss the grief that menopause can trigger for some women, the significant impact that menopausal drepession can have on careers and relationships and how medical experts need to work together to provide more holistic treatment plans for women. This chat with Dr Marita Long ticked a bucket list for me and brings a long list of important topics to the table that I know you will love and learn so much from! Marita was open to answering all of my questions, was honest about gender bias and education gaps that exist in the field of primary care. And then gives briliant tips for every woman to consider when it comes to talking to her GP about perimenopause and menopause. Dr Marita Long is a GP based in Victoria, Australia with a special interest in womens health and medical education. Marita sits on the Board of the Australasian Menopause Socity (AMS) and has an interest in Dementia education and is co-host of a podcast called Dementia in Practice. Marita's top tips: When booking an appointment let them know that you are coming in to discuss perimenopause or menopause Ask for a long (30 minute) appointment Ask to see the Doctor that has an interest and experience with women's health If you see a GP that is not your regular family carer, ask for a summary letter from your appointment to take back to your family doctor to ensure continuity of care We talked about: AMS - Symptom Score Sheet Australsian Menopause Society (AMS) website Where to find Sonya: Instagram Facebook Take the Midlife Quiz Stellar Women Website Thank you for listening to my show!
