Fibroids are extremely common, especially in Black and Brown women. Over half of women will develop fibroids during their childbearing years. Everyone is offered a genetic screen called a carrier screening during pregnancy. The carrier screen detects if you are a carrier for 4 to 274 diseases, depending on the type of screen ordered. Well, one of those screens detects if you have Fragile X premutation. Fragile X syndrome is caused by CGG repeats on the X chromosome and causes mild to severe intellectual disability. It affects both males and females, but females usually have milder symptoms owing to the fact that the syndrome is inherited in an x-linked fashion. With the premutation, CGG repeats are present, but not enough for the baby to have the syndrome. A molar pregnancy is a rare complication that can happen when there are two many of dad’s chromosomes. This can happen when dad’s chromosomes duplicate because mom’s chromosomes are missing or two of dad’s sets of chromosomes are matched with one set of mom’s. This causes cells that would normally form the placenta to form cysts that grow throughout the uterus. The decision to have genetic screening to evaluate for genetic problems with your baby is a big decision. The most common type of genetic screen is what’s called a noninvasive prenatal screen (NIPS), which is a blood test that screens cells that enter your (mom) blood circulation from the placenta. NIPS is a pretty good test! It has a high detection rate for Down syndrome and a few other common genetic abnormalities. BUT, this test can screen for a lot of stuff… some of which are super rare. What’s the issue with that, you ask? Well, the sensitivity for the other conditions the test screens for is much lower, we’re talking anywhere from 40-80%, which is basically a coin toss in some situations. Spina Bifida is a defect in the spinal cord that leaves the spinal nerves exposed to amniotic fluid. Babies with spina bifida can have issues with walking and bowel and bladder function. Every November Prematurity Awareness Month is observed, with World Prematurity Day being recognized globally on the 17th. One common reason for a preterm delivery is when a patient's water breaks before being full term (or 37+ weeks pregnant). During pregnancy, the baby resides inside a fluid-filled sac, called an amnion which contains amniotic fluid. When someone says their water has broken, they really mean the membranes of the amniotic sac have ruptured. Rupture of membranes (ROM) after 37 weeks usually happens after the onset of labor (or the start of contractions). However, when this rupture occurs before 37 weeks, this is called preterm premature rupture of membranes (PPROM), and this can cause a world of trouble. Sickle cell anemia, also called sickle cell disease, is an inherited condition that causes sickling of red blood cells. These sickled cell aren’t able to carry oxygen effectively to tissues in the body. As a result of lack of oxygen to tissues and damage to vessels by the sickled cells, patients with the disease experience pain crises and organ damage (including heart attacks and strokes). Sickle cell anemia is most commonly found in African Americans and can cause several complications in pregnancy. About 1:12 Black people are asymptomatic carriers for the sickle cell trait and might not even know it! One of the strongest indications that there might be something going on with your baby is whether the skin behind the neck is thick. In the first trimester, your provider looks at what’s called the nuchal translucency, which is a space behind the neck and back. Half of babies with an increased measurement of this space have a chromosomal abnormality. Once you make it to 16 weeks, your provider looks at the nuchal fold, which is the skin behind the lower part of the skull. This is also highly associated with chromosomal abnormalities, with the most common being Down Syndrome and Turner Syndrome. Can genetically normal babies have thickened nuchal folds and translucencies? Of course! But this could also be a clue that there is something structurally going on as well. Every pregnant woman should get an ultrasound to look at their baby’s structures at approximately 18-20 weeks. During that ultrasound, not only are your baby’s organs closely evaluated but the placenta (or afterbirth) and the umbilical cord are also evaluated. Some babies with chromosomal abnormalities can look completely normal on ultrasound. So, as a high risk pregnancy physician, I look for what’s called soft markers (or subtle hints that something’s going on). One of these hints is dilation of your baby’s kidneys during pregnancy. Kidneys filter blood and rid waste of blood through urine. If urine is blocked inside of the kidney, they look enlarged (dilated) on ultrasound. This can sometimes be the only hint that there could be Down syndrome or another genetic problem. Other times, this can be something completely normal that resolves over time. Neonatal Jaundice, also known as hyperbilirubinemia, is a yellow discoloration of the white parts of the eyes and skin found in newborn babies due to high levels of bilirubin.
