Do an internet search for “fertility supplements”... yeah, maybe don’t. You can easily get overwhelmed by an avalanche of supplement information and advice when you’re trying to conceive. But … what supplements can truly help you get pregnant faster? And whose advice can you trust? It's not just the big traditional holidays that can be triggering when you’re struggling with fertility. It’s the softer, sneak-up-on you days too, Father’s Day being one of those. So often the female partner is the focus of the fertility journey, but the male partner is on the ride too. It can be harder for men to express their feelings and take space for what they need when they are wearing the care-taker/problem-solver hat in the relationship. So let’s make a little space for the “other” partner today. Dr. Lora Shahine opens up the conversation with her guest Dr. Erica Bove who is an OBGYN, Reproductive Endocrinologist and Fertility Coach. Dr. Bove wears her doctor coat and coaching badge simultaneously, which allows her to care for the patient but also help couples navigate the impact infertility can have on their relationship. What does advocating for reproductive rights have to do with IVF? Surely people who want to have a baby wouldn’t have to fight for that right in a post- Roe world? Well, as it turns out, the reality is that the science behind IVF doesn’t always mesh with the idea that life begins at conception. This week, Dr. Lora Shahine welcomes Barb Collura, President and CEO of RESOLVE, the National Infertility Association dedicated to advocating for fertility treatments and ensuring that building a family is attainable for all. In this episode, Barb shares her fertility journey, details the issues IVF and other reproductive rights are facing, and explains why fighting to protect IVF is necessary. Many in the reproductive space saw the writing on the wall long ago. Here, Barb and Dr. Shahine break down how the overturning of Roe vs. Wade has threatened the process of IVF. They also discuss how Personhood Bills in various states end up impacting IVF — intentionally or unintentionally. There is much more to the conversation between Barb Collura and Dr. Shahine, so make sure to come back for part two on the next episode of Baby or Bust! Candles can be romantic, even sexy. But ironically, some can make it harder for you to get pregnant. Many common candles can have an impact on fertility, so if you’re trying to build your family, you may want to reconsider how you set the mood. Candles and scents may impact fertility, but never fear: information is power. On this episode of Baby or Bust , you will learn what you need to know about fertility and candles. In this episode, Dr. Lora Shahine is chatting with Tristan Weholt, founder of Rox Hill Candle Co. Tristan is here to share her insight on parabens, paraffin wax, and the harmful chemicals that are present in some candles. Dr. Shahine and Tristan also discuss the differences between store-bought, mass-produced candles and those that are purchased through smaller companies – like knowing what ingredients are in the products you purchase. Protecting reproductive rights is more important now than ever. For National Infertility Awareness Week, here’s how you can help: This pineapple candle (citrus scent) was created in honor of infertility and reproductive rights. For many struggling with infertility, building a family may take more than two people. Egg donation, sperm donation, surrogacy or using a gestational carrier can be a beautiful way to build a family, but the process can be full of many questions and misconceptions along the way. Why do egg and sperm donors donate? Can third-party donors really be anonymous in this age of in-home genetic testing? Should parents be open with their children about their biological origins? If so, when is the right time to share or disclose this? Finally, is it possible a parent won’t bond with their baby if there’s no biological connection between them? In Part Two of their discussion on the myths and misconceptions around fertility, Dr Shahine and licensed therapist Lisa Schuman tackle questions about third-party family building. Lisa Schuman is an award-winning researcher, therapist, author, and expert in donor conception, surrogacy and fertility treatment. Her organization, The Center for Family Building, is dedicated to helping people have an easier journey to becoming parents. In this episode you’ll hear: [0:00] What is “third-party reproduction”? Trying is supposed to be the fun part, right? Ask anyone struggling with infertility and they may share that intimacy can start to feel like a job after a while. Many fertility patients have found that at-home inseminations can be an option for trying to conceive at home. At-home inseminations are not the same as intrauterine inseminations in a fertility clinic and cannot substitute for consults, testing, and guidance from a reproductive endocrinologist. However, they can be an option to consider as a first-line treatment, for those who have difficulty with intimacy, and for some people using frozen donor sperm from a sperm bank. On this episode of Baby or Bust , Dr. Lora Shahine is talking all about at-home inseminations with Mauren Brown, founder of Mosie Baby. The Mosie Baby Kit for At-Home Insemination is the first FDA-cleared insemination syringe kit, and it has helped over 100,000 people access a fertility option that can take place in the comfort of their own home. Dr. Shahine and Maureen discuss the inspiration of Mosie Baby, how the syringe is designed, who might benefit from this option, and the future of at-home insemination. In the wake of the Dobbs decision that effectively ended federal abortion rights protection under Roe v Wade, the fate of IVF and fertility treatments has been hanging in the balance. In Alabama in February/March of 2024, a decision on the rights of a fetus brought IVF to a halt. Was this was the “pro-life” movement had in mind? To prohibit people from building the families they envision? Fortunately, that outcome was “patched,” thanks to activism by reproductive rights’ advocates, among others. One of the most important voices in this ongoing conversation (and spoiler: this conversation didn’t start when Roe v Wade ended) is that of Elizabeth Carr, the first IVF baby in the United States. Elizabeth Carr, who has been “in the media since three cells old,” chose a career in journalism, and she is now fully in as an advocate for fertility treatments like IVF. PGT-A, PGT-M, PGT-SR, Carrier Screening, Karyotype — oh my! The options available for genetic screening in fertility testing and IVF treatment can get confusing. Understanding testing available to you BEFORE treatment as well as DURING the IVF process is key. These tests can decrease risk of disease and miscarriage and improve success rates with a single embryo transfer, but there are pros and cons for every intervention. It can be difficult to keep it all straight and overwhelming to know what to do. In this episode, Dr. Lora Shahine goes in depth on what tests are available, what they are screening for, who is a good candidate for genetic testing, and what your options are when the results come in. You’ll finish this episode understanding genetic testing and screening in fertility and IVF and knowing what questions to ask your doctor about what’s right for your personal situation. Infertility advocate and health enthusiast Adelaide White is an invaluable source of light and hope to so many on TikTok and Instagram, Dr. Shahine included. In this episode, the tables are flipped as Adelaide interviews Lora on All Things Fertility Journey, from the educational journey of a reproductive endocrinologist to the differences among the fertility treatment choices to the many causes of infertility to the heartbreak of recurrent pregnancy loss. Through it all, one message is clear: infertility and pregnancy loss are not your fault. Culturally, we have such a tendency to blame infertility on the partner who plans to carry the pregnancy, and that’s both wholly inaccurate and hugely unhelpful. Shame and guilt have no place in this process, and that’s why podcasts like Adelaide’s and Dr. Shahine’s exist, so please share widely to spread the word. In this episode you’ll hear: [0:00] Fertility treatments with a reproductive endocrinologist — what to expect [8:43] Fertility success rates with IVF [14:16] Fertility treatments and hormone levels [19:12] Fertility treatment options and miscarriage risk [22:51] Miscarriage causes, egg retrieval pain, and IVF success and why recurrent pregnancy loss is not your fault [28:06] Fertility treatments and when to seek medical help: is one year enough? Those undergoing IVF sometimes hear they shouldn’t exercise or they might put the process at risk. However, we know a sedentary lifestyle isn’t healthy for your fertility either. For many, exercise doesn’t just keep their bodies healthy, it’s also critical to managing stress, aiding with sleep, and managing mental health — which can also impact fertility. So how much exercise is too much when trying to conceive and in early pregnancy? In this episode, Dr. Shahine helps us understand the facts and decide what is right for your personal situation. We considered posting this episode on New Year's Eve, when toasting in the New Year is likely to happen, rather than New Year's Day, when many celebrants are waking up to a sore head, a fuzzy tongue, and second thoughts about that third round of shots. If you’ve ever wondered where Dr. Shahine gets her ferocity from, well, meet Charlene. Inspired by Julia Louis-Dreyfus’s Wiser Than M e podcast, Dr. Shahine interviews a woman she truly admires: her mother, Charlene Kelly Ewing. Despite being “all of 5 feet,” according to daughter Lora, Charlene meets every challenge head-on and, at age 79, still pushes herself to do what she’s afraid of. The diminutive Southern belle may have a soft voice, but Dr Shahine knows to listen: her mom has been a guiding force as well as her daughter’s biggest cheerleader. Whether she’s impressed by her daughter’s ability to watch gnarly medical procedures and still eat pizza, or her drive to help others through not only health care but also health education, Charlene Ewing is as inspired by her daughter as her daughter is by her mom. Have a listen, then give yourself the gift of talking with a woman you admire. Happy holidays from Baby or Bust! “Why didn’t I know all this already?” - Pretty much everyone who reads Toni Weschler’s book The stigma around women’s reproductive systems isn’t just an inconvenience, it removes a critical way women get insight into their bodies and health overall: period power. The menstrual cycle tells us a whole lot more than if we’re pregnant or not, or menopausal or not, says author Toni Weschler. Her seminal (oh, the irony of that adjective!) work, Taking Charge of Your Fertility, is all about understanding and taking charge of your reproductive health and being “cycle savvy.” In this episode, Dr. Lora Shahine and Toni Weschler talk about the origin of this amazing work. (Hint: it has to do with women being really angry.) And despite the title, it’s not all about helping people who are hoping to become pregnant. It’s about, says the author, “teaching women on a day-to-day basis what is going on with their body on that day” and taking advantage of all the insights into your health charting your cycle can give you. In this episode you’ll hear: [00:00] Intro [02:26] What is ‘Taking Charge of Your Fertility’ and why Toni ‘hates’ the title [04:44] Toni’s inspiration to write the book [09:50] The difference between the rhythm method and fertility awareness [11:22] The 3 signs of fertility explained [14:56] What the menstrual cycle can teach you about your health [18:51] Being a pioneer: How did Toni research for her book in 1990s? No two fertility journeys are alike, but Tara Lipinski’s experience is perhaps even more unique than most. The champion, gold-medal-ice-skater-turned-sports-analyst was 35 and still not necessarily ready to start building her family. So she decided to put her options “on ice” — literally preserving eggs and/or embryos until she and her husband Todd were ready. However, the process turned out to be more complicated than it appeared when a diagnosis of endometriosis sent them down a different path. What followed was two surgeries, four miscarriages, six failed transfers, and five years of treatment, hope, and heartbreak. Spoiler alert: perfect Georgie was born in 2023, so there is a happy ending, but the picture on Instagram of Tara, curled up on her couch, heart-broken and clutching pregnancy tests, sent waves through the fertility community. Here was a very public figure speaking very publicly about her struggles to build her family. Please listen and share to help us continue the conversation. In this episode you’ll hear: [00:00] Intro [07:74] Why Tara and Todd shared their story on their Unexpecting podcast [13:19] How did Tara’s athletic career prepare her and NOT prepare her for infertility [19:51] Reflecting on relationships through infertility, men vs. women on coping [30:02]When to share and not share the fertility journey [34:53] When we are surprised by infertility [38:01] Misconceptions and reality about surrogacy [46:12] Instagram Questions Amanda: Did you and Todd ever consider giving up? The good news: IUIs can work! Intrauterine insemination (sometimes laughingly referred to as “the turkey baster method”), or IUI, can help overcome issues like low sperm count or low sperm motility by getting sperm closer to where it needs to be around the time of ovulation. This method of dealing with infertility is also less expensive and much less invasive for the person hoping to carry the pregnancy. It can also be a great method if you’re planning on using donor sperm. The harder news to hear: it may not work the first time. That said, there are things you can do to increase your chances of a successful IUI, and in this episode of Baby or Bust, Dr. Shahine goes into your many options, both medical and behavioral. In this episode you’ll hear: [00:00] Intro to the Episode [01:27] IUI review – what it is, how it works, how is it different from IVF [04:31] Tips BEFORE you do your first IUI Tip #1 Get testing before you start Tip #2 Review your family goals with your doctor Tip #3 Make a long-term plan – how many IUIs? Ep 47: Weight and Fertility: Optimizing Heath NOT Restriction and Shame with Dr. Stephanie Fein Weight loss for fertility is controversial and complicated, and to be perfectly honest, more research is needed to fully understand the link between body weight and fertility. Whatever studies find, weight loss for fertility should never involve shaming or guilt. In this episode, Dr. Shahine talks weight and fertility with Stephanie Fein, MD. Dr. Fein started in social psychology, then moved to Internal Medicine, where she completed a residency program. She then went on to obtain a Masters in Public Health. All of these skills came together when Dr. Fein was moonlighting at a weight-loss clinic while doing her residency; a conference on weight and fertility solidified her decision to move into helping people optimize their fertility with weight loss. What does the research really show? Perhaps most important to understand, no two bodies are the same, and many of the numbers and other information you’ll hear on the Internet (and maybe even from your doctor), won’t apply for all. In this episode you’ll hear: Time Stamps [00:00-03:18] Intro to the show and Dr. Stephanie Fein [03:19-07:27] Dr. Fein’s inspiration from medical doctor to Weight Loss for Fertility [07:28-16:43] Research on weight and fertility – Why BMI as a marker? Holiday events can mean facing some awkward questions about family building - are you ready? “So, when are you having another?” “You had a baby? It’s about time!” “When are you going to make your mom a grandma?” “Baby weight, huh?” “Should you be drinking that when you’re breastfeeding?” UGH. Holiday conversations — especially when alcohol and holiday conviviality reduces people’s inhibitions — can be kind of awful. So this year, please put yourself first. Dr. Shahine has some great tips on how you can do that. From finding an ally (hint: it doesn’t have to be your partner, who is likely fighting their own conversational battles) to designing conversational strategies from polite deflection to well-deserved zingers, being prepared in advance can really help in the moment. In this episode, Dr. Shahine explores the benefits and research supporting Yoga for Fertility with master instructor and author of the first book on the subject, Lynn Jensen. People on fertility journeys often say they feel like their bodies don’t belong to them anymore. Between the tests and the treatments and the theories, it’s easy to see how people can feel disconnected physically and emotionally. Lynn Jensen’s Yoga for Fertility classes helps heal bodies AND minds. Lynn has a specialization in yoga for the fertility journey, so her students can trust that they’re maximizing their physical and emotional wellness. Lynn is a Registered Yoga Teacher through the Yoga Alliance and a Certified Yoga Therapist through the International Association of Yoga Therapists since 2013. Not only does Lynn have highly specialized training, she’s been through the journey herself. Her classes often turn into impromptu counseling sessions, and her students make lifelong bonds with her and each other. In this podcast, Lynn and Dr Shahine talk about the many ways yoga heals and strengthens the body for fertility and the mind for the challenges of the journey. It’s easier to be shocked by the story The Retrievals podcast reports and moves on but extremely important to reflect on why it happened in order to prevent history repeating itself (again). For months, a nurse at a Fertility Center in the United States replaced the pain reliever fentanyl with normal saline, taking the fentenyl for her own use, but left women undergoing egg retrievals without pain relief. Patients in pain for a procedure is horrible enough, but it’s extremely disturbing that the women who tried to report and question what happened felt like their concerns were ignored. In this episode of Baby or Bust, Dr. Lora Shahine takes us through what an egg retrieval process should be like and is like, the majority of the time. Dr. Shahine also explores the history of women and the medical community and why women so often have to advocate for themselves to get even a minimum of care. She leaves with tips on how to advocate for yourself so you receive the attention and care you deserve. Listen and share! Take a moment to follow the show and leave a review wherever you listen to podcasts! Ovarian Hyperstimulation Syndrome (OHSS) is one of the scariest risk factors of IVF, but it doesn’t have to be. With preparation, treatment choices, and prevention, you can avoid this uncomfortable and potentially dangerous risk factor from ovarian stimulation. To maximize the number of eggs retrieved for IVF and egg freezing, fertility treatment involves stimulating the ovaries to mature more eggs. A higher egg number can often predict a higher success rate with treatment, but a side effect can be significant fluid shifts and bloating also known as OHSS. When OHSS occurs, it can be mild, moderate, or severe, with symptoms ranging from discomfort and bloating to significant dehydration, shortness of breath, nausea, and even hospitalization for recovery. In this episode, Dr. Shahine does a deep dive into OHSS. You'll learn What OHSS is Why OHSS happens and Who is at risk Ways to prevent OHSS Ways to treat symptoms if it occurs so you have a speedy recovery This week’s fertility story is about an OHSS patient Dr. Shahine will never forget from her training. It’s a reminder to never to ignore your care team’s instructions. In this episode you'll hear: [00:00] Intro: What is OHSS and why is it scary? [01:07] My experience caring for patients with OHSS over the years [02:13] OHSS Definition, Classification [05:29] Physiology of OHSS – Why does it happen? [07:12] Risk factors for OHSS – Who is at risk? [09:41] Treatment options to prevent and recover quickly from OHSS [09:55] Prevention of OHSS What to know before you start stimulation Choosing a protocol and medications to add to decrease risk Metformin, trigger shot, cabergoline, and more [21:47] Treatment if OHSS happens How to recover faster Increasing electrolytes and protein at home, When do you need IV fluids or hospitalization to recover (rarely) [25:58] Fertility story of Dr. Shahine’s Patient: Warning of the dangers of OHSS [31:51] Outro and thank you Resources mentioned: Try LMNT daily during your stimulation process to increase electrolyte intake and decrease the symptoms of bloating during IVF and egg freezing.
